Hiatal hernia is due to a long problem with the gastrointestinal problem but it is not known to cause halitosis directly. Having a hiatal hernia means that you may be having reflux into your esophagus which can cause the halitosis. But it’s hard to imagine that you had a hernia for 40 years without any symptoms other than halitosis. Correcting the hernia may help with the halitosis but halitosis is usually a long standing problem that requires a lot of care as I’m sure you know. I hope that your hernia repair does help though.
