A pulmonary embolism is a blood clot that has traveled from one part of the body (usually the legs), to the lungs. These blood clots are caused by poor circulation in the legs. There are many causes of thrombosis in the legs; sitting for long periods of time (such as long plane or car trips), problems with the clotting mechanism in the blood, and old age. Middle age people and the elderly sometimes develop a condition called atherosclerosis; blood vessels that are normally elastic and stretchy, become hardened and less elastic, interfereing with the circulation of blood in the legs. When the blood cannot move through these vessels, a clot will form. Over time, this clot may eventually break loose, travel through the veins and back to the lungs, causing a pulmonary embolism. Some preventative measures include getting enough exercise (to keep your circulatory system in good shape), quit/cut back on smoking, limit the amount of fat and cholesterol in your diet, and if you travel long distance, stretch your legs several times every 20-30min and, if possible, get up and walk every half hour or so.

This is due to the sudden blood clot in the veins of the heart. This may be due to various reasons. Also this could be avoided by having a healthy living habits and regular work outs, or physical activities can help to avoid the risk of the disease.
