What is Vidalista?

Vidalista is a prescription medication. Its active ingredient is tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor.

It is indicated for the treatment of erectile dysfunction (ED) in adult men. It is also indicated for the treatment of the signs and symptoms of benign prostatic hyperplasia (BPH). Under physician direction, tadalafil may be used off-label in the management of chronic pelvic pain associated with prostatitis.

Vidalista is available in a wide range of doses, from 2.5 mg to 60 mg, and in high-potency formulations such as Vidalista Black 80 mg. Doses exceeding 20 mg per day are not approved by the U.S. Food and Drug Administration (FDA).

Dosing and Administration

The dosing regimen for Vidalista must be determined by a qualified healthcare professional based on the individual’s indication, renal and hepatic function, and concomitant medications.

Standard Dosing Regimens

For erectile dysfunction (ED), the recommended as-needed dose is 10 mg, taken at least 30 minutes prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. The maximum dosing frequency is once per day.

For daily use in ED or for benign prostatic hyperplasia (BPH), the recommended dose is 2.5 mg or 5 mg taken orally once daily, without regard to timing of sexual activity. For a comparison of dosing strategies, see this guide to daily vs. as-needed dosing.

Important Considerations

  • Vidalista may be taken with or without food.
  • Patients should be aware of the prolonged duration of action (up to 36 hours for as-needed dosing) and should not exceed one dose within this timeframe.
  • Dose adjustment is recommended for patients with severe renal or hepatic impairment.

Side Effects of Vidalista

Adverse reactions to tadalafil are dose-dependent and generally mild or moderate in nature. The following lists are organized by frequency.

Adverse Reactions Associated with Tadalafil Use
FrequencyReported Side Effects
Common (≥2%)Headache, dyspepsia, back pain, myalgia, nasal congestion, flushing, pain in extremity.
Less Common (1-2%)Dizziness, gastroesophageal reflux disease, visual disturbances, nausea.
Serious (Seek Immediate Medical Attention)
  • Priapism: Prolonged erection (>4 hours).
  • Sudden hearing loss or decrease, with or without tinnitus and dizziness.
  • Non-arteritic anterior ischemic optic neuropathy (NAION): sudden vision loss in one or both eyes.
  • Signs of cardiovascular events (e.g., chest pain, shortness of breath).
  • Severe hypersensitivity reactions.

Discontinuation of therapy should be considered if sudden auditory or visual disturbances occur.

Indications and Usage

Vidalista (tadalafil) is indicated for the following conditions in adult men:

FDA-Approved Indications

  • Erectile Dysfunction (ED): For the treatment of ED.
  • Benign Prostatic Hyperplasia (BPH): For the treatment of the signs and symptoms of BPH. For comprehensive information on BPH treatment options, refer to this guide on BPH medications.
  • ED and BPH: For the treatment of ED and the signs and symptoms of BPH when the conditions coexist.

Off-Label Use

The use of tadalafil for chronic prostatitis/chronic pelvic pain syndrome is off-label. This use should only be initiated and monitored by a physician with expertise in managing this condition.

Interactions Alcohol:

Use with caution
Concurrent use of alcohol and tadalafil may potentiate orthostatic hypotension, leading to increased dizziness, tachycardia, and headache. Patients should be advised of this potential interaction.

Interactions Other Medications:

Consult your doctor
Concomitant use of tadalafil with nitrates is contraindicated due to the risk of severe, life-threatening hypotension. Concomitant use with alpha-blockers requires careful dose titration and blood pressure monitoring. Potent CYP3A4 inhibitors significantly increase tadalafil exposure. A complete review of all concomitant therapies by a prescriber is mandatory. Refer to this guide to tadalafil interactions for detailed information.

Special Groups Pregnancy:

Unsafe
Vidalista is not indicated for use in women.

Special Groups Breastfeeding:

Unsafe
Vidalista is not indicated for use in women.

Special Groups Elderly:

Safe if prescribed
No dose adjustment is required solely based on age. However, greater sensitivity to the hypotensive effects and a higher incidence of comorbidities may warrant caution. A cardiovascular evaluation is recommended prior to initiation.

Special Groups Children:

Unsafe
Vidalista is not indicated for use in pediatric patients.

Effects on Activities Driving:

Use with caution
Due to potential side effects such as dizziness, blurred vision, or fainting, you should not drive or operate a vehicle until you know how Vidalista affects you personally.

Effects on Activities Operating Machinery:

Use with caution
For the same reasons as driving, exercise extreme caution when operating heavy machinery or performing tasks that require full concentration and steady coordination after taking this medication.

Important Safety Concerns

Cardiovascular Risk

Sexual activity poses a cardiac risk for patients with pre-existing cardiovascular disease. Physicians should consider the patient’s cardiovascular status before initiating treatment for erectile dysfunction. Tadalafil is contraindicated in patients for whom sexual activity is not advised due to underlying cardiac conditions.

Ophthalmologic and Otologic Effects

Non-arteritic anterior ischemic optic neuropathy (NAION) has been reported post-marketing in patients taking PDE5 inhibitors. Patients with a history of NAION in one eye are at increased risk of NAION in the other eye. Sudden decrease or loss of hearing has also been reported. The causal relationship with tadalafil use is not established but cannot be ruled out.

High-Dose Formulations

Formulations of Vidalista exceeding 20 mg (e.g., 40 mg, 60 mg, Vidalista Black 80 mg) are not approved by the FDA or EMA for the treatment of erectile dysfunction. These doses are associated with a higher incidence and severity of adverse reactions, including profound hypotension and priapism. Their use should only be considered under exceptional circumstances with direct medical supervision.

Warnings and Precautions

Cardiovascular Effects

Tadalafil has systemic vasodilatory properties and may cause a transient decrease in blood pressure. It is contraindicated in patients using any form of organic nitrate. Caution is advised when co-administering with antihypertensives or alpha-adrenergic blocking agents. Patients with left ventricular outflow obstruction or severely impaired autonomic control of blood pressure may be particularly sensitive to these effects.

Priapism

Prolonged erection exceeding 4 hours requires immediate medical intervention to avoid permanent damage to erectile tissue. Patients with anatomical deformation of the penis or conditions that may predispose them to priapism (e.g., sickle cell anemia, multiple myeloma) should use tadalafil with caution.

Renal and Hepatic Impairment

In patients with severe renal impairment (CrCl < 30 mL/min) or severe hepatic impairment (Child-Pugh Class C), the maximum recommended dose is 10 mg, not to be taken more than once every 72 hours. Use in end-stage renal disease requiring dialysis is not recommended.

Concomitant Use with Other PDE5 Inhibitors

The safety and efficacy of combinations of tadalafil with other PDE5 inhibitors have not been studied. Such combinations are not recommended.

Dosage and Administration

The following table summarizes the dosing for FDA-approved indications. Dosage must be individualized by a healthcare provider.

Recommended Dosing for Vidalista (Tadalafil)
IndicationRecommended RegimenMaximum Recommended Dose
Erectile Dysfunction (as-needed)10 mg prior to sexual activity (range: 5–20 mg).20 mg once per day.
Erectile Dysfunction (daily)2.5 mg once daily.5 mg once daily.
Benign Prostatic Hyperplasia5 mg once daily.5 mg once daily.
ED and BPH5 mg once daily.5 mg once daily.

Dose Modifications

  • Renal Impairment: In patients with severe renal impairment, the maximum recommended dose for as-needed use is 10 mg every 72 hours. For daily use, the recommended dose is 2.5 mg.
  • Hepatic Impairment: In patients with severe hepatic impairment, the maximum recommended dose is 10 mg every 72 hours. Daily dosing is not recommended.
  • Concomitant Medications: When used with potent CYP3A4 inhibitors, the maximum recommended dose is 10 mg every 72 hours for as-needed therapy, or 2.5 mg for daily therapy.

Drug Interactions

Tadalafil is primarily metabolized by CYP3A4. Concomitant use with drugs that affect this enzyme system may alter its plasma concentration.

Clinically Significant Drug Interactions
Interacting Drug ClassPotential EffectClinical Management
NitratesProfound hypotension.Contraindicated.
Alpha-BlockersAdditive blood pressure lowering.Initiate tadalafil at 2.5 mg once daily if co-administration is necessary. Monitor blood pressure.
Potent CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir)Increased tadalafil exposure.Limit tadalafil dose to 10 mg every 72 hours (as-needed) or 2.5 mg (daily).
AntihypertensivesAdditive blood pressure lowering.Monitor for hypotension.
AlcoholPotential additive hypotensive effects.Advise caution.
Vidalista and Cialis share the same active pharmaceutical ingredient, tadalafil. Vidalista is a generic version, while Cialis is the reference listed drug (brand name). Both are required to meet standards for bioequivalence established by regulatory authorities.
Following a single as-needed dose of 10 mg or 20 mg, the plasma half-life of tadalafil is approximately 17.5 hours, with effects potentially persisting for up to 36 hours. This pharmacokinetic profile distinguishes it from other PDE5 inhibitors with shorter half-lives.
Doses of tadalafil exceeding 20 mg per day are not approved by the FDA for the treatment of erectile dysfunction. The use of 40 mg, 60 mg, or 80 mg tablets is associated with a non-linear increase in adverse event incidence and severity. These formulations should only be used under exceptional circumstances with close medical supervision.
The absorption rate and extent of tadalafil are not altered by food intake. It may be taken without regard to meals.
Yes. Vidalista is a prescription-only medication in most jurisdictions. It should be obtained through a licensed pharmacy with a valid prescription following a medical evaluation by a qualified healthcare provider.
