What is Vidalista?

Vidalista is a prescription medication. Its active ingredient is tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor.

It is indicated for the treatment of erectile dysfunction (ED) in adult men. It is also indicated for the treatment of the signs and symptoms of benign prostatic hyperplasia (BPH). Under physician direction, tadalafil may be used off-label in the management of chronic pelvic pain associated with prostatitis.

Vidalista is available in a wide range of doses, from 2.5 mg to 60 mg, and in high-potency formulations such as Vidalista Black 80 mg. Doses exceeding 20 mg per day are not approved by the U.S. Food and Drug Administration (FDA).

Dosing and Administration

The dosing regimen for Vidalista must be determined by a qualified healthcare professional based on the individual’s indication, renal and hepatic function, and concomitant medications.

Standard Dosing Regimens

For erectile dysfunction (ED), the recommended as-needed dose is 10 mg, taken at least 30 minutes prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg based on efficacy and tolerability. The maximum dosing frequency is once per day.

Important Considerations

• Vidalista may be taken with or without food.
• Patients should be aware of the prolonged duration of action (up to 36 hours for as-needed dosing) and should not exceed one dose within this timeframe.
• Dose adjustment is recommended for patients with severe renal or hepatic impairment.

Vidalista (tadalafil) is indicated for the following conditions in adult men:

FDA-Approved Indications

• Erectile Dysfunction (ED): For the treatment of ED.

Off-Label Use

The use of tadalafil for chronic prostatitis/chronic pelvic pain syndrome is off-label. This use should only be initiated and monitored by a physician with expertise in managing this condition.

Important Safety Concerns

Cardiovascular Risk

Sexual activity poses a cardiac risk for patients with pre-existing cardiovascular disease. Physicians should consider the patient’s cardiovascular status before initiating treatment for erectile dysfunction. Tadalafil is contraindicated in patients for whom sexual activity is not advised due to underlying cardiac conditions.

Ophthalmologic and Otologic Effects

Non-arteritic anterior ischemic optic neuropathy (NAION) has been reported post-marketing in patients taking PDE5 inhibitors. Patients with a history of NAION in one eye are at increased risk of NAION in the other eye. Sudden decrease or loss of hearing has also been reported. The causal relationship with tadalafil use is not established but cannot be ruled out.

High-Dose Formulations

Formulations of Vidalista exceeding 20 mg (e.g., 40 mg, 60 mg, Vidalista Black 80 mg) are not approved by the FDA or EMA for the treatment of erectile dysfunction. These doses are associated with a higher incidence and severity of adverse reactions, including profound hypotension and priapism. Their use should only be considered under exceptional circumstances with direct medical supervision.

Warnings and Precautions

Cardiovascular Effects

Tadalafil has systemic vasodilatory properties and may cause a transient decrease in blood pressure. It is contraindicated in patients using any form of organic nitrate. Caution is advised when co-administering with antihypertensives or alpha-adrenergic blocking agents. Patients with left ventricular outflow obstruction or severely impaired autonomic control of blood pressure may be particularly sensitive to these effects.

Priapism

Prolonged erection exceeding 4 hours requires immediate medical intervention to avoid permanent damage to erectile tissue. Patients with anatomical deformation of the penis or conditions that may predispose them to priapism (e.g., sickle cell anemia, multiple myeloma) should use tadalafil with caution.

Renal and Hepatic Impairment

In patients with severe renal impairment (CrCl < 30 mL/min) or severe hepatic impairment (Child-Pugh Class C), the maximum recommended dose is 10 mg, not to be taken more than once every 72 hours. Use in end-stage renal disease requiring dialysis is not recommended.

Concomitant Use with Other PDE5 Inhibitors

The safety and efficacy of combinations of tadalafil with other PDE5 inhibitors have not been studied. Such combinations are not recommended.

Dose Modifications

• Renal Impairment: In patients with severe renal impairment, the maximum recommended dose for as-needed use is 10 mg every 72 hours. For daily use, the recommended dose is 2.5 mg.
• Hepatic Impairment: In patients with severe hepatic impairment, the maximum recommended dose is 10 mg every 72 hours. Daily dosing is not recommended.
• Concomitant Medications: When used with potent CYP3A4 inhibitors, the maximum recommended dose is 10 mg every 72 hours for as-needed therapy, or 2.5 mg for daily therapy.

Drug Interactions

Tadalafil is primarily metabolized by CYP3A4. Concomitant use with drugs that affect this enzyme system may alter its plasma concentration.

Store at 25°C (77°F); excursions permitted to 15–30°C (59–86°F). Keep in the original container to protect from moisture and light.
