Exercise has always been a cornerstone of blood pressure management, but a new analysis suggests the type of workout you choose may matter more than previously thought.
A pooled analysis published in the British Journal of Sports Medicine found that several forms of exercise — particularly aerobic exercise, combined aerobic and resistance training, and high-intensity interval training (HIIT) — were tied to meaningful drops in blood pressure measured across a full 24-hour period.
The findings strengthen the case that regular physical activity can play a central role in managing hypertension, a condition that affects nearly half of adults in the United States.
Why 24-Hour Blood Pressure Readings Matter
The analysis focused on ambulatory blood pressure monitoring (ABPM) — a method that records blood pressure continuously throughout daily activity and sleep.
Unlike a single reading taken in a clinic, ABPM captures how pressure shifts across an entire day and night. Many specialists regard it as one of the most reliable ways to evaluate hypertension, because it reflects real-world fluctuations and nighttime patterns that a one-off measurement can miss.
A Head-to-Head Look at Exercise Types
Researchers pooled 31 randomized controlled trials involving more than 1,345 participants and 67 different exercise programs, each lasting at least four weeks.
The interventions spanned a wide range of activity:
- Aerobic exercise, such as walking, running, and cycling
- Resistance training with weights or body weight
- Combined aerobic and resistance exercise
- High-intensity interval training (HIIT)
- Isometric exercises, including planks and wall sits
- Pilates and yoga
- Recreational sports such as soccer and handball
Across every approach studied, aerobic exercise delivered the most consistent and reliable reductions in blood pressure during both daytime and nighttime hours.
Combined Training and HIIT Stood Out
Combined aerobic and resistance training produced one of the largest drops in systolic blood pressure — the top number, which reflects the pressure inside arteries while the heart pumps.
Compared with no exercise:
- Combined training lowered systolic pressure by an average of 6.18 mm Hg
- HIIT lowered it by 5.71 mm Hg
- Aerobic exercise lowered it by 4.73 mm Hg
For diastolic blood pressure — the lower number, measured between heartbeats — the biggest reductions came from:
- HIIT, at 4.64 mm Hg
- Pilates, at 4.18 mm Hg
- Combined training, at 3.94 mm Hg
- Aerobic exercise, at 2.76 mm Hg
These figures may look modest, but even small, sustained reductions in blood pressure can meaningfully lower the long-term risk of stroke, heart attack, and heart failure.
Why Aerobic Exercise Keeps Winning
Researchers say aerobic exercise likely works through several cardiovascular mechanisms at once. Sustained movement improves blood vessel flexibility, supports endothelial function — the ability of vessels to widen and narrow as needed — and lowers resistance throughout the circulatory system. Together, these effects ease the pressure inside the arteries.
Aerobic exercise also rests on the strongest evidence base, backed by a large number of trials that show consistent benefits.
Where Resistance Training Fits
Resistance exercise stays important for overall health, especially for preserving muscle strength and physical function with age. But the researchers suggest it works best as a complement rather than a stand-alone treatment for hypertension. Lifting can briefly spike arterial pressure during exertion, which may help explain why aerobic training showed steadier reductions overall.
HIIT for People Short on Time
HIIT alternates short bursts of intense effort with recovery periods. Researchers note it may be especially practical for adults who can’t fit longer sessions into a busy schedule.
Because HIIT workouts tend to be shorter than steady continuous exercise, they may also improve long-term adherence — one of the biggest factors in keeping blood pressure under control. Even so, experts caution that more large-scale studies are needed before HIIT can be recommended as a first-line treatment.
Activities That Still Need More Evidence
Yoga, Pilates, and recreational sports showed some promise, though the evidence was less consistent. Many of those studies involved smaller groups or incomplete reporting on adherence and side effects, so their long-term effect on blood pressure remains less certain.
Building Exercise Into Long-Term Blood Pressure Care
Experts stress that consistency matters more than intensity. For most adults with hypertension, the most practical plan is a sustainable routine built around aerobic exercise, with resistance training added for broader health benefits.
Whatever the format — walking, cycling, interval training, or recreational sport — the routines people can actually maintain over time are the ones most likely to produce lasting improvement.
Exercise won’t replace medication for everyone with hypertension. But regular physical activity remains one of the most effective lifestyle tools for lowering blood pressure and protecting long-term cardiovascular health.
