As the use of weight-loss medications continues to grow, researchers are increasingly examining their effects beyond weight reduction. One area receiving greater attention is bone health, particularly among people with type 2 diabetes, who may face an increased risk of fractures during weight-loss treatment.
New findings presented at ENDO 2026, the Endocrine Society’s annual meeting, suggest that semaglutide may offer an unexpected advantage. Compared with several other weight-loss medications, semaglutide was associated with greater weight loss and a lower rate of bone fractures.
The results have not yet been published in a peer-reviewed journal.
Why Bone Health Matters During Weight Loss
Weight loss can provide important health benefits for people with type 2 diabetes, including improved blood sugar control and reduced cardiovascular risk. However, losing weight can also accelerate bone loss in some individuals, potentially increasing the risk of fractures.
Researchers therefore wanted to determine whether semaglutide, a widely prescribed GLP-1 receptor agonist used for diabetes management and chronic weight management, affects fracture risk differently than other medications.
Comparing Semaglutide With Other Weight-Loss Treatments
The study analyzed data from the Atropos Health Eos electronic health record database, which contains information from healthcare systems across the United States.
Researchers identified adults with type 2 diabetes who:
- Had no prior history of fractures
- Were not receiving osteoporosis treatment
- Had received medication intended to support weight loss
The semaglutide group was compared with patients taking one of several alternative therapies:
- Phentermine/topiramate
- Bupropion/naltrexone
- Dulaglutide
After matching participants based on factors such as age and body mass index (BMI), each group included 17,506 individuals. A subset of 4,191 matched pairs had detailed BMI data available for analysis.
Greater Weight Loss and Fewer Fractures
The researchers found that semaglutide users experienced the largest reductions in BMI over time.
More notably, fracture rates were lower among people receiving semaglutide.
Over an average follow-up period of more than three and a half years:
- The semaglutide group experienced 794 fractures
- The comparison group experienced 1,045 fractures
Based on these findings, researchers estimated that semaglutide was associated with a 15% lower incidence of fractures compared with the other medications evaluated.
The results suggest that semaglutide may not carry the same concerns about bone loss often associated with weight reduction and may potentially provide some degree of skeletal protection.
Important Limitations
Experts emphasize that the study should be interpreted cautiously.
Because the research relied on retrospective electronic health record data, researchers could not control for every factor that influences fracture risk. Variables such as physical activity, dietary habits, bone density, fall risk, and other medical conditions may have affected the results.
Additional limitations include:
- Missing BMI information for many participants
- Data drawn primarily from academic medical centers and community hospitals
- A mixed comparison group consisting of medications with different mechanisms of action
- Analysis of fractures as a single combined outcome rather than by fracture type
As a result, researchers cannot determine whether semaglutide affects hip fractures, spinal fractures, wrist fractures, and other injuries differently.
Does Semaglutide Protect Bone?
At present, the answer remains unclear.
The study suggests that semaglutide users experienced fewer fractures despite greater weight loss, a finding that warrants further investigation. However, experts caution that the study cannot determine whether semaglutide directly improves bone health or whether the observed benefit reflects other factors.
Randomized clinical trials that include measurements of:
- Bone mineral density
- Bone turnover markers
- Specific fracture outcomes
will be necessary before researchers can conclude that semaglutide has true bone-protective effects.
Potential Clinical Implications
If future studies confirm these findings, they could influence treatment decisions for people with type 2 diabetes who are at increased risk for fractures.
Clinicians may become more confident that semaglutide-associated weight loss does not inherently increase fracture risk. The medication could potentially become a preferred option for certain patients, particularly older adults and postmenopausal women, populations already vulnerable to bone loss.
Still, experts stress that the current evidence is not strong enough to change clinical practice.
For now, semaglutide remains one of several effective tools for managing type 2 diabetes and obesity. Whether its effects on bone health are independent of weight loss remains an open scientific question.
Researchers say the findings are best viewed as an important starting point for future studies rather than definitive proof of a protective effect.
