Medical records are requested for review prior to your visit. You will meet with Dr. Hayes to go over your medical history and previous treatment. You will have a physical examination. You will then meet with a Registered Nurse to schedule further testing. At time of visit bring all related insurance information, drivers license/or picture identification. If you have an HMO, a referral with authorization is required.
