Toni Plummer is a Nurse Practitioner who specializes in psychiatric and mental health care and engages in a partnership with individuals to provide person-centered care. Person-centered care seeks to empower individuals to be active participants in their own mental health care rather than passive observers. Toni will focus on helping you to take control of your mental health. She’s been working in mental health since 1993 and has been privileged to partner with individuals of all ages in both community and hospital settings. Ideas such as acceptance, mindfulness, and distress tolerance have been crucial in Toni’s work with individuals regardless of their diagnosis. People experience trauma in many different ways; Toni understands the complexities of arousal states, grounding skills, triggers, and creating safe spaces. “I will assess how stress from all areas of your life contributes to your current mental health symptoms and help you identify areas where alterations could help …improve your mental health,” says Toni. “We will use evidence-based assessment tools to assist in diagnosing your mental health diagnosis. Understanding your diagnosis can help you make sense of your current symptoms and aid in discerning appropriate treatment options. Our time together will be also be spent on my providing education on medication treatment options that research has shown are effective in treating your specific diagnosis. We will discuss medication options with consideration of your financial, social, religious, cultural, and transportation resources to create a plan that works for you. I will monitor you regularly for any side effects or changes needed in dosage.” We all struggle at some point in our lives and none of us are born with a book on how to manage our own mental health.
