Key Facts
- The most common side effects of ED pills are mild, temporary, and a direct result of the medication working as intended.
- Headaches, facial flushing, and a stuffy nose are the “big three” because the pills relax blood vessels throughout the body, not just in the penis.
- Serious side effects are extremely rare, but you must know what they are.
- An erection lasting more than four hours (priapism) is a medical emergency.
- Most men find the benefits of treatment far outweigh the minor inconvenience of these temporary side effects.

The “Price of Admission”: Why Side Effects Happen
When a man decides to start treatment for erectile dysfunction, the first question is always “Will it work?” The second is, “What’s the catch?” It’s a question about side effects, and it’s a smart one to ask. I think of the common side effects as the “price of admission.” You’re getting a significant benefit, and sometimes there’s a small, temporary cost.
Here’s the simple science behind it. All the main ED pills—Viagra, Cialis, and the others—are called vasodilators. Their job is to relax and open up blood vessels to improve blood flow. They are fantastic at this. The thing is, they can’t *only* target the blood vessels in the penis. They have a mild, system-wide effect. And that’s what causes most of what you might feel.
The Usual Suspects: Common and Mild Side Effects
Most of the side effects men experience are no big deal. They are predictable, they don’t last long, and they usually lessen the more you use the medication. Here are the most common ones.
| What You Might Feel | Why It Happens & My Usual Advice |
|---|---|
| Headache | This is number one. The blood vessels in your head are opening up a bit, changing the pressure. My advice: make sure you’re well-hydrated. Drinking a big glass of water when you take the pill can often prevent it or lessen it. |
| Facial Flushing | Your face feels warm and gets a bit red. Again, it’s just increased blood flow to the skin. It’s harmless and fades as the medication wears off. |
| Nasal Congestion | Feels like you have a cold or allergies. The blood vessels in your nasal passages are swelling slightly. A simple saline nasal spray can help if it bothers you. |
| Indigestion / Upset Stomach | The muscle at the top of your stomach can relax a bit, sometimes leading to heartburn. Taking the pill with a light snack or using an over-the-counter antacid usually solves it. |
| Muscle Aches / Back Pain | This one is a signature side effect of tadalafil (Cialis). We don’t know exactly why it happens, but it’s temporary and harmless. It usually shows up a day after taking it and is gone within two days. |
The Red Flags: Rare But Serious Side Effects
Okay, let’s get serious for a moment. The side effects above are manageable annoyances. The next two are extremely rare, but they are true medical emergencies. You need to know the difference.
- Priapism: An Erection That Won’t Go Away. If you have a firm erection that lasts for more than four hours, this is not a good thing. This is a dangerous condition where blood is trapped in the penis and can cause permanent tissue damage. This is a ‘drop everything and go to the emergency room’ situation.
- Sudden Vision or Hearing Loss. This is exceptionally rare, but if you experience a sudden, significant change in your vision in one or both eyes, or a sudden loss of hearing, stop taking the medication and seek immediate medical help.
I tell you this not to scare you, but to empower you. Knowing what to watch for is the key to using these medications safely.
My Bottom Line: Don’t Be Afraid, Be Informed
The overwhelming majority of my patients who take ED medication find that the benefits to their confidence and relationships are life-changing. For them, a mild headache for an hour or two is a tiny price to pay. The key is knowing what’s normal and what isn’t. If any side effect—even a mild one—is bothering you, talk to me. Sometimes a simple dose adjustment or switching to a different pill in my ED medication comparison toolkit is all it takes to find the perfect fit.
