For most men, this is where we start. The pills known as PDE5 inhibitors are safe, effective, and easy to use. What if pills don’t work, or you can’t take them due to other health conditions? This is where many men think their options run out, but that’s not true. Penile injections are an incredibly effective second-line treatment.
I know what you’re thinking: a needle? Yes. But it’s a tiny needle, like an insulin syringe, and the injection into the side of the penis is virtually painless. It delivers medication directly where it needs to go, causing a powerful, reliable erection within minutes, without needing sexual stimulation. The most common options are:
- Alprostadil (Caverject, Edex): A single, FDA-approved medication that is very effective at widening blood vessels.

A pump (either manual or battery-powered) creates a vacuum, which pulls blood into the penis. A constriction ring is then slipped onto the base of the penis to hold the erection. They are effective, safe, and non-invasive.
- Penile Implants: This is a surgical solution and is typically reserved for when other treatments have failed. An implant is a device placed entirely inside the body that allows a man to have an erection whenever he chooses. The satisfaction rates for both patients and their partners are extremely high. It is a permanent and highly effective fix.
A Critical Warning: The Dangers of Online ‘Cures’
You will see countless ads online for “herbal Viagra” or other supplements promising to cure your ED. Please, be extremely careful. These products are not regulated by the FDA. At best, they are ineffective sugar pills. At worst, they can contain dangerous, unlisted ingredients, including illegal doses of prescription ED drugs or even contaminants. Products like Kamagra, for example, are illegal in the US for this very reason. Always get your treatment through a licensed medical professional.
