Key Facts
- There is no single “best” ED pill. The best one is the one that fits your life.
- All major ED pills (sildenafil, tadalafil, etc.) are in the same class of drugs and are highly effective at improving blood flow.
- The main differences are timing: how fast they work and, more importantly, how long they last.
- Your lifestyle, relationship, and how much spontaneity you want are the key factors in choosing the right pill.
- Safety is paramount. They all share one critical rule: they can NEVER be mixed with nitrate medications for chest pain.

It’s About the Right Tool for the Job
Almost every man who comes to my office for help with erectile dysfunction asks the same question: “Doc, which pill is the best?” It’s a fair question, but it’s the wrong one. The right question is, “Which pill is best for me?”
I want you to think of these medications like different types of vehicles. Sildenafil (Viagra), tadalafil (Cialis), vardenafil, and avanafil (Stendra) are all made by the same “manufacturer”—they’re all in a class of drugs called PDE5 inhibitors. They all do the same fundamental job: they help relax the blood vessels in the penis to allow for better blood flow. But you wouldn’t use a sports car to haul lumber, and you wouldn’t use a pickup truck for a racetrack. Each is designed for a different purpose.
Meet the Contenders: The Four Main Players
Let’s line them up and see what they’re built for. This isn’t about which one is “stronger,” but about how they fit into your life.
| The Pill | The Nickname | The Gist (How it works in the real world) |
|---|---|---|
| Sildenafil (Viagra) | “The Original” | This is the most famous one for a reason. It’s reliable. You take it about an hour before sex, and it gives you a solid 4-6 hour window of opportunity. It’s perfect for a planned date night. |
| Tadalafil (Cialis) | “The Weekend Warrior” | This is the marathon runner. One pill can last up to 36 hours. This doesn’t mean a 36-hour erection, but rather the ability to respond to stimulation anytime within that window. It offers incredible spontaneity. You can read more in my guide to Cialis. |
| Vardenafil | “The Underdog” | It’s very similar to Viagra in its timing (4-6 hours). For some men, it seems to have slightly fewer visual side effects (like a bluish tinge to vision) and can be a great alternative if Viagra isn’t a perfect fit. |
| Avanafil (Stendra) | “The Sprinter” | Its claim to fame is speed. It’s the fastest-acting of the bunch, sometimes working in as little as 15 minutes. For men who value a very quick onset, this is an excellent choice. |
The Food Factor: A Quick But Important Note
This is a practical tip that matters. Viagra and Vardenafil can be slowed down by a heavy, fatty meal. If you have a big steak dinner and then take a Viagra, it might take a lot longer to kick in. Cialis and Stendra are not really affected by food, which adds to their flexibility.
The Big Safety Question: Are They Safe?
Yes, for the vast majority of men, these medications are extremely safe. They have been used by millions for over two decades. Their side effects are usually mild—things like headache, flushing, or a stuffy nose. I cover these in more detail in my article on ED medication side effects.
But there is one absolute, critical, non-negotiable safety rule. I say this to every single patient, and I’ll say it here in bold: NEVER, EVER mix these pills with nitrate drugs. Nitrates (like nitroglycerin) are often used for chest pain or angina. The combination can cause a catastrophic drop in your blood pressure that can be fatal. I will always review your full medication list before prescribing, which is why you must never take these pills without a doctor’s supervision.
So, What’s the Verdict?
The verdict is that you have fantastic options. The choice comes down to a conversation about your life.
- Do you usually plan intimate moments? Viagra or Vardenafil might be perfect.
- Is your sex life more spontaneous? Cialis, either the long-acting pill or the low daily dose, is likely your best bet.
- Do you want the fastest possible option? Stendra is your go-to.
My job isn’t to tell you which pill is best. My job is to explain the map and help you choose the vehicle that’s right for your journey. The conversation starts here.
